Basic Education
For HS Applicants
New Students
- First Year
- Need to pass the entrance test
- Card or Form 138 with Principal’s signature
- Certificate of good moral behavior
- Birth certificate
- Transferees
- No failing grades or dropped subjects
- Need to pass the entrance test
- Card or Form 138 with Principal’s signature
- Certificate of good moral behavior issued by the Principal from his/her previous school
- Will be accepted under certain terms of conditions or by signing the probationary form
- Birth certificate
Old Students
- Need to pass all their subjects
- Form 138-A
- For student with foreign family names they need to bring their birth certificate and/or Alien Certificate of Registration
- Certificate of good moral behavior
For Special Science Class only
- Weighted Average Grade of 85.00 or higher
- No grades lower than 80.00 in any subject except in Math, Science, and English (85.00 or better)
- Photo copy of Report Card – F138
Child Study Center
- Submit birth and baptismal certificate.
- Submit medical certificate.
- Pass the entrance exam.
- Pass the interview.
- Be 4 years old by June of the incoming school year for Kindergarten, and 5 years old for Preparatory
Elementary School
- Submit Birth Certificate and Medical Certificate
- Submit report card (F138) duly signed by the Principal of the school last attended.
- Submit Good Moral Certificate
- Complete Kinder and/or Preparatory (for Grade I applicants).
- Be 6 to 7 years old in Grade I, and proportionately older in the succeeding grades.
